The shrine district fits in a long day trip from Tokyo, but adding the Oku-Nikkō highlands — the falls, the lake, the marshlands — really wants an overnight. Autumn foliage traffic on the Irohazaka switchbacks can eat hours; start early.
The Tobu line from Asakusa is the classic route — limited expresses take about two hours and Tobu's Nikkō passes bundle the local buses. JR Pass holders can route via Utsunomiya instead (and grab gyoza on the way back).
